What to bring and confirm before your visit
Ask your school or athletic program for its current physical examination form and any instructions about who may complete it. Confirm that the clinic can meet those requirements before booking. Bring the required forms, any sections the school asks you to complete beforehand and relevant health information for the clinician. Is a sports physical the same as a routine checkup?
The purpose of this visit is to evaluate school sports participation and address the required documentation. It does not replace ongoing primary care or every other health assessment an athlete may need.
